The story begins on the morning of Shelby Eatenton's wedding in the fictional town of Chinquapin Parish, Louisiana. At Truvy Jones's beauty salon, the women of the community gather to prepare for the big day. Truvy, a warm and witty salon owner, has just hired a nervous newcomer named Annelle Dupuy, who is fleeing an uncertain past involving a husband who may be in legal trouble. M'Lynn Eatenton, the devoted mother of the bride, arrives with her daughter Shelby, a young woman with Type 1 diabetes who has always been determined to live life on her own terms despite her health challenges. Also present are Ouiser Boudreaux, a cantankerous wealthy widow with a sharp tongue, and Clairee Belcher, the former mayor's widow and Ouiser's longtime friend who provides dry comic relief.

The wedding itself goes smoothly, but the central conflict soon emerges: Shelby, against medical advice, decides she wants to have a child. Her diabetes makes pregnancy extremely dangerous, and M'Lynn is terrified for her daughter's life.
